Teeth that are badly decayed, cracked below the gumline, infected, or still trapped in the bone require surgical removal. The service is available at our two offices in Suwanee, with sedation and general anesthesia available if you would prefer not to be awake.
Why Your Dentist Referred You to an Oral Surgeon

They refer the harder ones out, and the reason is volume: oral surgeons are in an operating room every working day, so a tooth that is fractured at the root, buried in bone, or sitting next to a nerve is a routine morning here rather than an unusual case.
The other common reason for a referral is anesthesia. Our surgical suites have full monitoring equipment, our surgical assistants are certified in oral surgery, including anesthesia, and we can offer IV sedation or general anesthesia.

When a Tooth Needs to Come Out

Decay that has destroyed too much of the tooth structure for a filling or a crown to hold. Once there is not enough sound tooth left to build on, a restoration will continue to fail.
Infection or Abscess
An infection at the root that has not cleared or has come back after root canal treatment. Removing the tooth removes the source.
Cracked or Fractured Teeth
A tooth split below the gumline or fractured through the root cannot be repaired. This is the most common reason a general dentist sends an extraction to us rather than doing it in-house.
Impacted Teeth
A tooth that never fully erupted and is still held in bone or gum tissue. Canines are the usual case after wisdom teeth, and they are sometimes exposed and guided into place rather than removed, depending on the plan.
A Failed Root Canal
When a root canal is unlikely to hold, removing the tooth and replacing it is often the more durable choice. We will tell you honestly which way we would go.
Crowding Before Orthodontics
Occasionally, an orthodontist needs space created before braces or aligners can move teeth into position, and asks us to remove specific teeth to make it.

What Happens During a Tooth Extraction

The first visit is an evaluation. We take images, and for teeth close to a nerve or a sinus, we may take a 3D cone beam scan so we can see exactly where the roots sit before we touch anything. We also review your medical history and medications because both affect how we plan your anesthesia.
Simple Extraction or Surgical Extraction
A simple extraction lifts out a tooth that is fully erupted and intact enough to grip. A surgical extraction is needed when the tooth is broken off, still under the gum or bone, or has roots that will not come out in one piece. That usually means a small incision in the gum, sometimes removing a little bone around the root, and sometimes sectioning the tooth so it comes out in pieces rather than forcing it whole. Sutures close the site afterward.
Anesthesia and Sedation Options
Local anesthetic alone is enough for many extractions. For longer procedures, multiple-tooth procedures, or patients who would prefer not to be aware of it, we offer IV sedation and general anesthesia, administered and monitored by our own team. If being awake for this is the part you are dreading, say so when you call, because it changes what we plan rather than something you have to endure.
Planning for What Replaces the Tooth
Once a tooth is out, the bone that held it starts to change shape, which can narrow the options for replacing it later. So at the consultation, we talk about what goes in the gap before we take anything out. Depending on the plan, that can mean placing a bone graft in the socket at the same appointment to preserve the site, then a single-tooth dental implant once it has healed.
Does getting a tooth pulled hurt?
Not during the procedure. The area is fully numb, and if you choose IV sedation or general anesthesia you will not be aware of it at all. What you feel is pressure and movement rather than pain. Afterwards there is soreness, and it usually peaks on the second or third day, which surprises people who assume day one is the worst of it.
How long does a tooth extraction take?
The appointment is longer than the extraction. A simple extraction is often a matter of minutes; a surgical one on a broken or impacted tooth takes longer, and multiple teeth in one sitting longer still. If you are having sedation, plan for the whole appointment plus recovery time and arrange a ride home.
Do I need a referral from my dentist?
No. Most of our extraction patients arrive on a dentist’s referral, but you can book an evaluation directly. If you do have a referring dentist, tell us who, and we will send them a report so your records stay in one place.
Can I drive myself home after the tooth removal procedure?
With local anesthetic only, yes. With IV sedation or general anesthesia, no, and we will not discharge you without someone to take you home.
